By checking the box and signing below, I hereby grant permission to Dèloiu Studio to use my likeness in photographs and/or videos in any and all of its publications, including but not limited to website entries,or any other lawful way we choose.
 

I understand and agree that these materials will become the property of Dèloiu Studio.

I hereby irrevocably authorize Dèloiu Studio to edit, alter, copy, exhibit, publish, or distribute these materials for purposes of publicizing [photographer's name or company]'s programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears.
 

I hereby hold harmless and release and forever discharge Dèloiu Studio from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorisation.
 

I confirm that I am over the age of 18 and have the right to contract in my own name. I have read and understood this release and fully understand the contents, meaning, and impact of this release.

I understand that by checking the box and signing below, I am giving my consent for the processing of my personal data, including my name, image, and likeness, in accordance with the General Data Protection Regulation (GDPR) and any other applicable data protection laws.
